Costos médicos directos en el tratamiento de Covid-19 durante el primer año de pandemia en la ciudad de Bogotá D.C

Abstract

La enfermedad SARS-COV-2/COVID-19 se caracteriza por síntomas de infección respiratoria aguda, como fiebre, dolor de cabeza, tos seca y dificultad para respirar; de igual manera puede involucrar el tracto gastrointestinal y el sistema nervioso. Solo algunos de los pacientes infectados se enferman gravemente requiriendo terapias invasivas con sedoanalgesia asistida por ventilación mecánica. El presente estudio tuvo como objetivo estimar los costos médicos directos en el manejo de las categorías clínicas (leve, grave y critica) de los casos confirmados de Covid 19 en la ciudad de Bogotá D.C, durante el primer año de pandemia en el periodo de marzo 2020 a marzo 2021. Se obtuvo la información proveniente de los Manuales Tarifarios SOAT 2020 – 2021 y el Sistema de Información de Precios de Medicamentos (SISMED), estableciendo así criterios de inclusión de eventos generadores basados en la Guía de la Asociación Colombiana de Infectología (ACIN) y bajo supuestos diseñados por conceptos médicos especializados en la enfermedad. Se determinó la distribución de la población en adulta y pediátrica (grupos etarios: 0-5 años y 11 a 15 años). Posteriormente se estimaron los costos totales del tratamiento del Covid 19, según la categoría clínica, evidenciando que la categoría grave fue la que genero un gasto superior al sistema de salud de la ciudad de Bogotá D.C, durante el periodo costeado con el 41,1% del costo ($347.034.129.932 COP); seguido de la categoría critica con un gasto del 30,2% ($254.934.477.842 COP); y la categoría leve aunque representa el mayor porcentaje de la población, fue la categoría que genero un menor gasto con el 28,7% ($242.876.753.410 COP). Finalmente se estimó el costo total del periodo establecido por un valor de $ 844.845.411.024 COP, donde el 98% ($827.835.754.052 COP) de los costos fueron generados por la población adulta y solo el 2% ($17.009.607.132 COP) la población pediátrica.

SARS-COV-2/COVID-19 disease is characterized by symptoms of acute respiratory infection, such as fever, headache, dry cough and shortness of breath; it may also involve the gastrointestinal tract and the nervous system. Only some of the infected patients become seriously ill requiring invasive therapies with sedoanalgesia assisted by mechanical ventilation. The aim of this study was to estimate the direct medical costs in the management of the clinical categories (mild, severe and critical) of confirmed cases of Covid 19 in the city of Bogotá D.C., during the first year of the pandemic in the period from March 2020 to March 2021. Information was obtained from the SOAT 2020 - 2021 Tariff Manuals and the Drug Pricing Information System (SISMED), thus establishing criteria for the inclusion of generating events based on the Guide of the Colombian Association of Infectious Diseases (ACIN) and under assumptions designed by medical concepts specialized in the disease. The distribution of the population into adult and pediatric (age groups: 0-5 years and 11 to 15 years) was determined. Subsequently, the total costs of the treatment of Covid 19 were estimated, according to the clinical category, showing that the severe category was the one that generated a higher expenditure to the health system of the city of Bogotá D.C., during the cost period with 41.1% of the cost ($347,034,129,932 COP). 034,129,932 COP); followed by the critical category with an expense of 30.2% ($254,934,477,842 COP); and the mild category, although it represents the highest percentage of the population, was the category that generated the lowest expense with 28.7% ($242,876,753,410 COP). Finally, the total cost for the established period was estimated at COP 844,845,411,024, where 98% (COP 827,835,754,052) of the costs were generated by the adult population and only 2% (COP 17,009,607,132) by the pediatric population.
